Many Medicare beneficiaries are surprised to learn that their Medicare premiums can be affected by their income. This adjustment, known as the Income-Related Monthly Adjustment Amount (IRMAA), applies to both Medicare Part B and Medicare Part D. The Social Security Administration determines IRMAA based on a beneficiary's modified adjusted gross income (MAGI) reported on their tax return from two years prior. As income rises above certain thresholds, additional monthly premium surcharges are added to Medicare coverage costs.
While IRMAA is often associated with high-income retirees, it can also affect individuals who experience significant one-time income events such as retirement account withdrawals, property sales, business transactions, or other unexpected increases in taxable income. In many cases, beneficiaries are unaware that these events can trigger higher Medicare premiums until they receive a notice from Social Security informing them of the adjustment.
Fortunately, not every IRMAA determination is final. Certain life-changing events—including retirement, reduction in work hours, loss of income-producing property, divorce, or the death of a spouse—may qualify a beneficiary for a review and possible reduction of their IRMAA assessment. Understanding how Medicare premiums are calculated and knowing when an appeal may be appropriate can potentially save beneficiaries hundreds or even thousands of dollars per year.
